1.2 Medical Disclaimer
This website provides educational information regarding sleep apnea, snoring, TMJ disorders, and airway-related health conditions. It is not a substitute for professional medical or dental advice. A doctor-patient relationship is established only following an in-person evaluation and signed consent.
1.3 Appointments, Cancellations, and Payments
- Appointments can be scheduled through the website or by calling the office.
- A 24-hour notice is required for cancellations. Missed appointments may result in a fee.
- Payment is due at the time of service, unless otherwise arranged.

- Any uncovered or out-of-network costs are the responsibility of the patient.
1.4 Insurance and Billing
The Center for Breath Treatment may accept or assist with select insurance plans for qualifying medical procedures. Patients should verify coverage with their providers before scheduling. Non-covered services are patient-responsible.
1.5 Emergency Disclaimer
This site does not provide emergency care. If you experience severe breathing distress, jaw pain, swelling, or medical emergencies, call 911 or go to your nearest emergency department.
1.6 Limitation of Liability
Dr. Steven Y. Park and the Center for Breath Treatment are not liable for any damages arising from the use of this website or reliance on its information, as permitted by California law.
1.7 Telehealth Consultations
Virtual consultations are provided for convenience but are not a substitute for in-person evaluation. Final diagnosis and treatment recommendations require an in-office assessment.
1.8 Minors
Treatment for minors requires the written consent of a parent or legal guardian, except as permitted under California law for emergencies.
1.9 Intellectual Property

2.1 General Treatment Consent
Before initiating any treatment, patients will receive written and verbal explanations outlining:
- The nature and purpose of the recommended therapy (oral appliance, TMJ adjustment, airway treatment, etc.)
- Possible risks, benefits, and alternative options
- Consequences of non-treatment
- Expected outcomes and maintenance requirements
- Financial and insurance responsibilities
2.2 Treatment-Specific Consent
2.2.1 Oral Appliance Therapy (Sleep Apnea & Snoring)
- Explanation of appliance design, fit, and expected use
- Disclosure of potential side effects (bite changes, discomfort, muscle tension)
- Follow-up schedule for adjustments and long-term monitoring
2.2.2 TMJ & Jaw Pain Therapy
- Discussion of imaging, diagnostics, and muscle assessment
- Disclosure of potential temporary discomfort during therapy
- Importance of compliance and follow-up
2.2.3 Airway and Myofunctional Treatment
- Explanation of exercises and oral posture correction
- Emphasis on patient participation and home care
- Expected duration and milestones of therapy
2.2.4 Surgical or Referral Procedures
If surgical intervention is required (ENT, sleep surgery, etc.), appropriate referrals and consent protocols will be followed.
2.3 Documentation
All informed consent discussions will be documented in the patient’s record, including date, provider, and patient acknowledgment.

3.1 Controlled Substance Compliance
The Center follows California Health and Safety Code §11159.2 and CDC prescribing guidelines.
3.2 Prescription Protocol
- Medications are prescribed only when medically necessary
- Limited duration and lowest effective dose
- Documentation of medical indication in patient records
- Patient education on proper use and disposal
3.3 Monitoring and Documentation
- Review of medical history and medication interactions
- Use of California CURES database for controlled substances

4.1 Patient Confidentiality

- CBCT, sleep studies, and radiographs stored securely and retained for minimum 7 years
5.4 Record Disposal
- Secure shredding and digital deletion protocols
- Documentation of disposal maintained
5.5 Transfers
- Patient-signed authorization required for all transfers
- Copies provided within 15 days of request
- Staff Training & Compliance
- HIPAA & OSHA training within 30 days of hire, and annually thereafter
- Sleep medicine and airway education annually for clinical staff
- Documentation of all continuing education and compliance audits
- Quality Assurance: Patient satisfaction reviews and case outcome tracking
- Privacy Policy (HIPAA & CCPA Compliant)
7.1 Information Collected

- Name, contact information, insurance details
- Medical and dental history
- Diagnostic data (CBCT, sleep tests, airway scans)
